  • Request for Decline in Value Review (Prop 8) – When the taxpayer believes the market value of the property on January 1st has suffered a decline in value below the assessed value, this form may be submitted to the Assessor’s Office so that the current assessed value may be reviewed. Select the appropriate form for the property type:

  • Change in Ownership Value Review
  • Assessment Appeal Application (BOE 305-AH) - This form can be accessed on the Clerk of the Board of Supervisors web page (the link is near the bottom of the page). This form is used to formally request an assessment change and is to be submitted to the Clerk of the Board of Supervisors, Assessment Appeals Board.
  • Property Damaged by Misfortune or Calamity - Allows adjustments to assessed value for properties damaged through no fault of the owner.

  • Church Exemption (BOE 262-AH)
  • Church Lessor's Exemption Claim (BOE 263-C) - Property leased by a church to a public school, community college, state college, or state university, including the University of California, used jointly with a church.
  • Property Use Report - Supplemental form to include with institutional exemption claims when the exempt owner allows others to use the property. Cannot be used with Welfare exemption claims.

  • Lessors' Exemption Claim (BOE 263) - To be filed by lessor. Property used for free public libraries and free museums and for property used exclusively for public schools community colleges, state colleges, state universities, University of CA, churches and nonprofit colleges. The benefit of the exemption must go to the exempt organization in the form of a rent reduction or direct refund, unless otherwise stated in the lease.
  • Qualified Lessors' Exemption (BOE 263-A) - To be filed by lessor. Claim for exemptions claimed on property used for free public libraries and free museums and for property used exclusively for public schools, community colleges, state colleges, state universities, University of California, and nonprofit colleges. For property which is being purchased with an agreement granting the lessee the option to acquire the leased property for one dollar or other nominal amount at the end of the lease.

  • Combination Request Form - This is an owner-initiated request to combine more than one parcel into a single parcel. (See our brochure entitled Split/Combination/LBA: Where Boundaries Change under Tax Cycle Information.)
  • Split Request Form - This is an owner-initiated request split one parcel into multiple parcels along historical lines. (See our brochure entitled Split/Combination/LBA: Where Boundaries Change under Tax Cycle Information.)
  • Application for Tax Segregation (SEPARATE VALUATIONS) - use to request the allocation of split values to facilitate payment of taxes
